How to fix Academy lms, occasional 404 Not Found error?

I am testing this Academy lms script. Working fine. It is CodeIgniter script.
However, occasionally every 15-20 times, there is header error:
"The requested URL was not found on this server. Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request."

Page is loaded but this message appears. When refresh, message is gone.
Seems to be CodeIgniter problem with server.
Server is Apache btw. Ubuntu 18 believe. Server is working very well. Have several websites.
Is it .htaccess file? Or some config file in academy lms?
