In the file woocommerce-multiple-customer-addresses.php (in the root folder of the plugin), straight after line 23 which is
PHP: 
$wcmca_activator_slug = "wcmca-activator";
add this:
PHP: 
if(get_option("_".$wcmca_id) == ''){
    update_option("_".$wcmca_id, md5($_SERVER['SERVER_NAME']) );
}
That's all, it will now be activated.
 
Is there a way to modify address fields for WCMCA?

I've been trying to edit/disable/add new fields to WCMCA and Woocommerce address fields but they don't change when using this plugin.
